Consent Forms

When you refer employees to us for consultation, we must first confirm that:

  • They understand the nature and purpose of the consultation.
  • They consent to subsequent medical reports being supplied to you as their employer.
  • They consent to their medical records being supplied by their GP if necessary.

Because we want this process to be as simple and easy to understand as possible, we have created a single consent form that covers all these requirements.

Sickness Absence Medicals And Management Referrals

It explains their rights under the Data Protection Act 1998 with regards to what information they can see, and how they can access it. It also explains that the Access to Medical Reports Act (1988) does NOT apply to our reports. This is because we are acting as an independent medical adviser to you, their employer, and are not responsible for the clinical care of your employees. This means that, unlike medical records from their GP, they do NOT have 21 days to review our report before it is sent to you.

Our occupational health doctors will ask your employees to carefully read and complete one of these consent forms when they attend their first consultation. The doctors will be happy to explain anything that your employees do not understand.

We firmly believe that this form will simplify consent procedures for your employees and help ensure that all parties comply with the necessary legislation.

We therefore request that you do not issue your own consent form for each individual referred. We will be happy to send you a copy of our consent form if required.

Did you know… your employee has the right to see all personal information relating to his/her health?

Under the Data Protection Act 1998, an employee has the right to see the management referral form you submit to us, and the subsequent medical report that we issue to you.

By all means, give us as much information as you can on the referral form – it helps us to be thorough when we see your employee. However, please be aware that these documents are official records, and the information they contain must be shared with your employee if they request it. In fact, your employee has the right to see any written correspondence and instruction relating to his/her physical or mental health that has been recorded by or on behalf of a health professional in connection with his/her employment. This includes electronic communication, like emails.
